Psychiatric Care

Medication Management

  • Diagnostic evaluation: Comprehensive assessment of symptoms, medical history, current medications, substance use, and relevant lab results to determine appropriate pharmacologic strategies.

  • Medication selection and initiation: Evidence-based choice of psychotropic medications tailored to the diagnosis, symptom profile, comorbid medical conditions, age, pregnancy/breastfeeding status, and patient preferences.

  • Dose titration and monitoring: Ongoing follow-up to adjust dosages for effectiveness and tolerability, monitor side effects, and manage drug interactions.

  • Safety monitoring: Regular assessment for adverse effects, metabolic labs when indicated (e.g., lipids, glucose), weight checks, and suicide risk evaluation. Coordination with primary care for medical monitoring.

  • Long-term management and deprescribing: Periodic reassessment of need for continuation, strategies for tapering when appropriate, and relapse prevention planning.

Biopsychosocial Approach

Medication-assisted treatment for opioid use disorder

We offer Medication-Assisted Treatment (MAT) for opioid use disorder, commonly known as Suboxone (buprenorphine/naloxone). MAT is provided as part of a comprehensive treatment plan that includes medical monitoring, counseling, and support for long-term recovery.

Medical cannabis certification

We provide evaluation and certification for medical marijuana cards where clinically appropriate and permitted by state law. Our clinicians assess eligibility based on current evidence and individual patient needs.

Whitney Onstad is a Board-Certified Psychiatric Mental Health Nurse Practitioner serving the community of Kauaʻi. With 16 years of nursing experience on the island, Whitney brings deep roots, clinical expertise, and a genuine commitment to whole-person care to her psychiatric practice.

Whitney approaches mental health through a biopsychosocial lens — recognizing that biological, psychological, and social factors all shape wellbeing. She works collaboratively with each client to develop an individualized treatment plan that feels right for their unique life, goals, and circumstances.
